CD Evaluation Purpose

The purpose of a chemical dependency evaluation is to assess whether an individual has a substance use disorder, determine the severity of the issue, and understand how substance use affects their daily life. This evaluation helps to identify the specific needs of the individual and the impact of their substance use on their relationships, work, and overall health. Based on the findings, the evaluation guides professionals in recommending the most effective treatment options to address any underlying mental health or behavioral issues contributing to the substance use.

During the CD Evaluation

During a chemical dependency evaluation, you can expect a personal interview about your substance use history, health, and how it impacts your life. You may complete questionnaires or screening tools to assess the severity of the issue. The evaluator will observe your behavior and provide treatment recommendations, such as counseling, outpatient therapy, or inpatient rehab, based on the findings. The process is confidential and aims to create a plan to support your recovery.

CD Evaluation Costs

A chemical dependency evaluation is $300. This fee covers a comprehensive assessment conducted by a licensed professional to determine whether an individual has a substance use disorder. The process involves reviewing the individual’s history of substance use, their mental and physical health, and any related personal, social, or legal challenges. Based on the findings, the evaluation provides recommendations for appropriate treatment options, such as counseling, outpatient therapy, or inpatient rehabilitation, to support recovery.

After the CD Evaluation

After a chemical dependency evaluation, you’ll receive a treatment plan with recommendations such as counseling, therapy, or rehab. You may be referred to specific programs or specialists. Follow-up sessions may occur to track progress and adjust the plan as needed. The evaluation results are confidential.

Chemical dependency evaluations with Creative Counseling and Studio require a 14-day turn around for report writing. If you need your evaluation sooner than 14-days, an additional fee of $200.00 will apply.
